What is actually wrong with it

The fault list is short. A running tank and a weak flush are both usually parts inside the tank. A leak at the base is the one that damages the building, and a fixture that rocks is the one that causes it.

Ask which part was tested before the fixture was blamed. On a running toilet that means the dye test; on a leak at the base it means whether the flange was looked at. A quote to reset a toilet that does not mention the flange has not established why the last seal failed.
